Service Agreement

This Service Agreement ("Agreement") is entered into between Servant's Heart Enterprise, LLC ("Provider"), owned and operated by Sheila Armstrong, and the Client and/or Client's Authorized Representative ("Client") identified at the time of service enrollment.

Servant's Heart Enterprise, LLC provides non-medical, faith-based in-home caregiving services, which may include but are not limited to:

• Personal care assistance (bathing, grooming, dressing)
• Companionship and emotional support
• Meal assistance — warming up meals and mealtime support
• Light housekeeping and laundry
• Medication reminders (non-medical)
• Transportation to appointments and errands
• Overnight and live-in care
• Respite care for family caregivers

The specific services to be provided will be outlined in the individualized Care Plan agreed upon by both parties prior to the start of services.

Payment terms, billing schedule, and accepted payment methods will be agreed upon and documented in the individualized Care Plan prior to the start of services.

Service hours and schedule will be established in the individualized Care Plan. Changes to the schedule must be communicated at least 24 hours in advance when possible. Emergency schedule changes should be communicated as soon as practicable.

Either party may cancel scheduled services with a minimum of 24 hours' notice. Repeated late cancellations may result in a review of the service agreement. Servant's Heart Enterprise reserves the right to cancel services in cases of unsafe working conditions or non-payment.

• Provide a safe and respectful working environment for caregivers
• Communicate care needs, preferences, and changes promptly
• Ensure timely payment as agreed in the Care Plan
• Notify Provider of any changes in the client's health or medical condition
• Provide necessary supplies and equipment as agreed

• Provide compassionate, professional, and reliable care
• Maintain client confidentiality and privacy at all times
• Communicate any concerns or changes in the client's condition to the family
• Arrive on time and fulfill agreed-upon service hours
• Treat all clients with dignity, respect, and Christian love

Servant's Heart Enterprise, LLC agrees to keep all client information strictly confidential. Personal, medical, and financial information will not be shared with any third party without the written consent of the client or their authorized representative, except as required by law.

Servant's Heart Enterprise, LLC provides non-medical caregiving services only. The Provider is not responsible for medical decisions, diagnoses, or outcomes. In the event of a medical emergency, the Provider will contact emergency services (911) immediately and notify the client's emergency contact.

Either party may terminate this agreement with a minimum of 7 days' written notice. Immediate termination may occur in cases of abuse, unsafe conditions, theft, or other serious violations. Upon termination, all outstanding balances are due immediately.

By engaging the services of Servant's Heart Enterprise, LLC, the Client acknowledges that they have read, understood, and agree to the terms outlined in this Service Agreement. A signed copy of the individualized Care Plan and Agreement will be provided to both parties prior to the start of services.
